Add ci_commit_status() and ci_pipeline_number() helpers to
lib/ci-helpers.sh that query Woodpecker directly with a forge API
fallback. Replace all 12 inline forge commit status calls across 6
files with the new helpers.
Add setup_woodpecker() to bin/disinto init that creates a Forgejo
OAuth2 app for Woodpecker and activates the repo.
Document manual Woodpecker+Forgejo setup in BOOTSTRAP.md.

- Remove existing TOML files from git tracking (git rm --cached)
- Add .toml.example templates with placeholder values

Existing local installations are unaffected — gitignore only
prevents tracked files from being committed, local copies persist.

- dev-agent.sh: add explicit guard that skips formula-labeled issues with a
clear log message instead of silently producing no formula behavior
- BOOTSTRAP.md: rewrite formula label entry to state it is not yet functional
and that dev-agent will skip such issues until feat/formula is merged

The supervisor agent was confusingly named "factory" (same as the
project). Rename directory, script, log, lock, status, and escalation
files. Update all references across scripts and docs.
FACTORY_ROOT env var unchanged (refers to project root, not agent).
